What Is a RACI Matrix
Clarifying Roles and Responsibilities in Safety Management and ISO 45001 Systems
A RACI Matrix is a role clarification tool used to define and document responsibilities across tasks, projects, or organisational systems. It maps who is Responsible, Accountable, Consulted, and Informed for each activity.
In safety critical environments, particularly those aligned with ISO 45001 Occupational Health and Safety (OH&S), a RACI Matrix helps ensure that duties are clearly defined and that all stakeholders understand their role in managing workplace risk.
Understanding the RACI Roles
- Responsible (R): The person or team tasked with completing the work or performing the activity.
- Accountable (A): The individual ultimately answerable for the outcome and approval of the work.
- Consulted (C): Subject matter experts or stakeholders who must provide input before the task is completed.
- Informed (I): Individuals who must be kept aware of progress or outcomes but are not directly involved in the work.
Why Use a RACI Matrix in OH&S?
Within ISO 45001 aligned safety systems, the RACI Matrix is commonly used to clarify responsibilities for safety related tasks, audits, inspections, emergency planning, and risk control activities.
- Clarifies who performs actions and who approves decisions in risk management workflows.
- Prevents duplication of effort and confusion around responsibilities.
- Strengthens stakeholder engagement by identifying who should be consulted or informed.
- Supports ISO 45001 Clause 5.3 regarding organisational roles, responsibilities, and authorities.
Example: RACI Matrix in a Permit to Work System
In high risk operations, a RACI Matrix might be used to define responsibilities within a permit to work process.
- Responsible: Supervisor issuing or controlling the permit.
- Accountable: Site manager responsible for overall safety compliance.
- Consulted: Safety officer providing technical or regulatory input.
- Informed: Workers affected by the task or operating nearby.
Implementing RACI in Safety Programmes
RACI Matrices are often developed during process mapping or stakeholder analysis workshops. They can be used alongside tools such as IPO Analysis, Stakeholder Mapping, or Onion Diagrams to build a comprehensive view of responsibilities and system interactions.
